Highest Quality Service/Performance Improvement

HHCA's mission is to provide the highest quality skilled home nursing service. Towards that end, the Company has designed a Performance Improvement ("PI") Program to effectively measure, assess and improve the care and services provided to the Company's patients. Its main objectives are:

  • Standardize the processes and systems necessary to provide quality patient care throughout HHCA.
  • Assure that individualized patient care and service are coordinated among the appropriate disciplines throughout the organization.
  • Monitor utilization and outcome data.
  • Assess and maintain the clinical proficiency of staff.
  • Maintain complete and accurate documentation of care and service provided.

The PI Program is based on a systematic, inter-disciplinary and thorough evaluation of community needs, referral source needs, staff feedback, financial constraints, customer satisfaction data as well as current standards of practice. It is designed to reflect HHCA's mission, and is a dynamic and flexible program designed to respond to the changing healthcare climate. Performance measurement is done on both process and outcome indicators.

Each HHCA office has a PI committee or team consisting of members from all disciplines in the organization. The committee reviews specific audit results and PI projects, patient satisfaction data, and incident and infection reports and makes recommendations for change or improvement, then follows up by measuring the effectiveness of the actions taken. This employee involvement empowers the individuals and promotes interdepartmental teamwork throughout HHCA. One example of HHCA's PI Initiative was recently featured in the JCAHO Resources 2001 publication "Cost-Effective Performance Improvement in Home Care". PI team members in HHCA's Florida West Coast region identified an opportunity to improve the timely return of the signed physician plans of care ("POC"). HHCA's management then expanded the regional initiative to the rest of HHCA's Florida offices to improve the POC process. A statewide PI task force designed and executed a plan that increased POC compliance from 70% to 90% within 21 days of start of care. The initiative was cited by JCAHO as a "real world example" of how a home care organization improved key processes cost-effectively.

To further enhance its clinical capabilities, HHCA utilizes a Corporate Medical Director who is responsible for the overall quality of all medical and related technical aspects of the Company. Dr. Ian M. Ballard has served HHCA in this advisory capacity for over 20 years. During that time, he has provided consultation and guidance to Company personnel and referring physicians regarding professional issues, worked on the development and continued refinement of the Company's policies and procedures and clinical programs, and helped HHCA educate its communities about home health issues. In addition to being a member of the Company's Professional Advisory Committee, Dr. Ballard is also a member of HHCA's Board of Directors.
